What is deprescribing ? Is it just about stopping pills ? Clearly not ! This session will review the goals and process of deprescribing in the older patient and includes the presentation of 1) a systematic patient-centered approach to deprescribing, and 2) evidence-based tools to help identify deprescribing opportunities and promote patient engagement throughout the process. 

Objectives

  • Plan a deprescribing process for community-dwelling older adults
  • Identify evidence-based tools to overcome barriers to deprescribing 
  • Present a systematic patient-centered approach to deprescribing using clinical case studies
